Background and Aims Knee osteoarthritis (KO) is a debilitating disorder. Limited studies have shown that Prolotherapy with the injection of hypertonic saline, instead of dextrose injection, is effective in diabetic patients and reduces the negative effects of this disease
Methods In this randomized clinical trial, 40 patients with KO and effusion were divided into two groups of 20. The first group received both peri-articular and intra-articular injection of hypertonic saline. The second group received only intra-articular hypertonic saline injection. Both groups received injections at 3 sessions with a 2-week interval. The Western Ontario and McMaster Universities Arthritis Index, visual analogue scale, and the Oxford knee score were used before treatment, 2 weeks after, and 4 weeks after the last session of injection for both groups. Data was analyzed using SPSS software, version 25 and paired T- test and independent T- test.
Results Although both groups were improved in all outcomes following intervention, there was a significant difference between the two groups in the improvement level in favor of the first group 1 that received both intra- and peri-articular injections.
Conclusion It seems that simultaneous peri- and intra-articular hypertonic saline injection leads to better results in patients with KO and joint effusion than the intra-articular injection only.
